Supplemental Pleadings: Uniform Child Custody Jurisdiction and Enforcement Affidavit (UCCJEA)

This document is based on the Uniform Child Custody Jurisdiction and Enforcement Act. This is a Uniform Act drafted by the National Conference of Commissioners on Uniform State Laws in 1997 and has been adopted by the state of Florida. This document helps the court establish when, how, where and if a child custody case/parental responsibility and timesharing case can be established. It contains a sworn statement of all the addresses in which a child has lived at for the past 5 years. It also includes the people that lived with the child at that address.
